OrderSurvey is a Shopify application that enables merchants to create and display post-purchase surveys on thank-you and order status pages. The service includes survey creation, response collection, analytics, and data export.
To use OrderSurvey, you must have an active Shopify store and install our app through the Shopify App Store or Partner dashboard. By installing, you agree to these terms.
Charges are processed through Shopify and appear on your Shopify invoice. You may cancel at any time through your Shopify admin billing settings.

You retain ownership of all survey data collected through your store. We act as a data processor on your behalf. See our Privacy Policy for details on how data is handled.
We strive to maintain high availability but do not guarantee uninterrupted service. We may perform maintenance or updates that temporarily affect availability.
OrderSurvey is provided "as is" without warranties of any kind, express or implied. To the maximum extent permitted by law, we shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, or business opportunities, arising from your use of the service.
Our total liability for any claim arising from or related to the service shall not exceed the amount you paid us in the 12 months preceding the claim.
You may terminate your use of OrderSurvey at any time by uninstalling the app from your Shopify store. We may suspend or terminate your access if you violate these terms. Upon termination, your data will be deleted in accordance with our Privacy Policy.
We may update these terms from time to time. Continued use of the service after changes constitutes acceptance of the updated terms.
